Is this a terrible idea? RR Spartacus mount with factory tailgate hinges. I still have the stock mount, so may go back to that. Will I need to extend the bumper things with a -19 offset wheel and 31” tire.

As long as you extend the bumpers to press against the tire it will be fine. Even with the stock hinges that spare tire isn't big enough to put too much stress on the hinges I'd think.
Of course it looks pretty stupid with how far it sticks out, but that's merely my opinion.

I had the Spartacus tire carrier and went back to the stock one for this very reason. I hated how far it pushed the tire out.
It has to be meant for some seriously massive (wide?) tires to have that much space. Even with my 35x12.5 tires it still had an atrocious amount of gap.
