Parking brake adjuster failure

Hi all,
1999 TJ. I just rebuilt the rear drum brakes with all new hardware including new parking brakes cables and adjuster. I was fine tuning the parking brake today and pulled up on the lever and the cable popped out of the adjuster. On closer inspection, the cable barrel has deformed the adjuster and pulled straight out of the adjuster housing. Am I doing something wrong? Wrong cable barrel or wrong adjuster? Or is this a faulty adjuster w weak metal?

More info:
I bought the Dorman cable part # C95344 (https://www.rockauto.com/en/moreinfo.php?pk=359737&cc=1431777&pt=1696&jsn=12). I'm pretty sure there's only one way to install this cable, and both ends have the same type of barrel.
The adjuster, or cable equalizer is the ULTRA-POWER 926284 (https://www.rockauto.com/en/moreinfo.php?pk=14596137&cc=1431777&pt=16491&jsn=10).
I'm thinking I should just buy the Dorman adjuster (https://www.rockauto.com/en/moreinfo.php?pk=11724729&cc=1431777&pt=16491&jsn=9). I'd assume they are more compatible.

I installed the Dorman adjuster and same problem- the barrel of the cable slips right through. So must be the cable is wrong. Everything else with the cable works fine, just seems the barrel is too small for the adjuster/equalizer holes.
Any thoughts on next steps? A different cable? A fix that enlarges the existing barrel? Something else? I’d prefer to not have to replace the whole cable as I don’t want to tear into the drum brakes again, but if I have to so be it.

Determine what the Outside Diameter of the cable is, purchase some thick washers which have a slightly larger Inside Diameter as the cable OD, cut notches in the washers to allow the washers to slide over the cable and slide the barrel into the adjuster/equalizer holes making sure the washers are between the barrels and the inside of the adjuster/equalizer.
This will stop the barrels from pulling thru the holes.
