Rear Axle Play Looks Bad, Am I Right?

I noticed this play in my rear while trying to address a separate issue. It is up and down/side to side play rather than the typical in and out play. Looks pretty bad to me. What do y’all think? Is there any way to tell whether it’s the axle or the bearing, without taking the axle out?
 
Axle Pulled. Does this look like an excessive amount of wear to you guys? The wear on the shaft alone doesn’t look like enough to account for the amount of slop between the axle and bearing when installed, so I’m assuming that I should at least replace the bearing, but I’m on the fence about swapping the axle shaft.

That is a zero wear surface. That groove is very bad.
