Ox locker keeps shearing off bolt head

Dana 44 4:88 ox cable locker 3 time it's sheared of 1 bolt head in 2 years can't figure out how this keeps happening any ideas?

Yep, he asked if I was sure if it was shimmed properly. I think so because the guy who did it builds race trucks for a living. I think it is vibration causing it but I'm not sure. I feel a little vibration around 65, 70 but so hard to tell running chewed up 35" tires.

It is likely a combination of two things, one for sure. The for sure one is your gear guy is not setting up enough pre-load on the carrier and the probably issue is a machining imperfection that causes that area to be just slightly higher between the two case halves.
